Make sure to enter the account number provided to you by your Pain Diagnostic and Treatment Center patient representative.

If you are unable to pay the patient responsibility estimate, please contact us directly: (916) 231-8755. A Pain Diagnostic and Treatment Center patient representative can assist you in setting up a payment plan prior to/on the date of service.
